Liability Release Agreement

By signing up for and attending classes, private sessions, and/or workshops or other services and offerings with Holistic Healing NJ and its affiliates, I acknowledge that I understand the potential risks involved. I understand that yoga includes physical movement, and as is the case with any physical activity, the risk of possible injury, even serious and disabling, is always present and cannot be entirely eliminated. I agree that if experience any pain or discomfort, I will listen to my body, adjust the posture or movement, and proceed at my own pace. Classes, private sessions, and/or workshops or other events with Holistic Healing NJ and its affiliates are not a substitute for medical care or evaluation. I understand that physical exertion is not recommended and is not safe under certain medical conditions. I hereby consent to receive medical treatment that may be deemed advisable in the event of injury, accident and/or illness during any classes, private sessions, and/or workshops or other offerings with Holistic Healing NJ and its affiliates. I affirm that I alone am responsible for deciding whether to take classes, private sessions, workshops, and/or other offerings with Holistic Healing NJ and its affiliates. If I am pregnant or become pregnant or am postnatal, my acknowledgment verifies that I am participating in offerings with my doctor's full approval.

I realize that I am participating in classes, private sessions, and/or workshops, or other offerings at my own risk. I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: I (a) irrevocably WAIVE, RELEASE, AND DISCHARGE FROM ANY AND ALL LIABILITY for my death, disability, personal injury, property damage, property theft or actions of any kind which hereafter may occur to me, including my traveling to and from classes/private sessions/workshops or other offerings, Holistic Healing NJ and its affiliates, who are hosting these classes and locations/facilities where sessions are being held, and each of their directors, officers, employees, volunteers, representatives and agents; and (b) INDEMNIFY, HOLD HARMLESS AND AGREE NOT TO SUE the entities or persons mentioned in this paragraph as to any and all liabilities or claims made as a result of participation in classes, private sessions, and/or workshops or other offerings, whether caused by the negligence of releases or otherwise. By signing up for classes, private sessions, and/or workshops or other offerings with Holistic Healing NJ and its affiliates, I hereby certify that I have read this document and I understand its content.
